How to get pensions benefits

 When a member is retiring or leaves employment for any reason and wishes to claim benefits the following steps are necessary:

Old Age (Retirement) Pension

An application form LAPF 12 should be duly filled by the employee in triplicate and certified by the employer. This form is available at the employer’s office or at any of LAPF Zonal offices. Supporting documents to be attached are:

  • A letter stating reasons for retirement
  • Two stamp size photographs of the claimant  
  • First employment/ Confirmation letter
  • Last salary slip
  • A copy of Membership card

Survivors’ benefits

Application form LAPF 13 providing information and particulars of the deceased depositor should be filled in triplicate. This form is available at the employer’s office or at any of LAPF Zonal offices or in this website soon. Supporting documents to be attached are:

  • A death certificate of the deceased depositor
  • Administrator’s copy (Certified by the Resident Magistrate)
  • Two stamp size photographs of the administrator of the deceased estate
  • Deceased contributor’s Membership card
  • First employment/Confirmation letter
  • Last salary slip

In case of Invalidity Pension

Application form LAPF 13 should be duly filled in triplicate with the following documents attached:

  • Medical Certificate (Certified by the approved Medical Board)
  • Employer’s certification letter
  • Copy of Membership card
  • Two stamp size photographs.

Withdrawal due to resignation, termination, dismissal, retrenchment and others:

Application form LAPF 14 should be duly filled in triplicate with the following documents attached:

  • Employer’s notification letter
  • Membership card
  • Two stamp size photographs.

In case of withdrawal due to emigration:

Application form LAPF 15 should be duly filled in triplicate with the following documents attached:

  • Residence Permit of the foreign country
  • Resignation letter certified by the employer
  • Membership card
  • Two stamp size photographs

Processing time

Upon receipt of the complete filled forms and attachment of all relevant supporting documents, the benefits will be paid within one month. In the next Financial Year, through improved technology all benefits will be paid on retirement date.

Where to get LAPF services

LAPF Head quarters are based in Dodoma. There are 6 Zonal offices across the country.

The Central Zone which is based in Dodoma provides Fund’s services for Singida, Tabora, Kigoma and Tabora Regions.

There is the Southern Highlands Zone in Iringa region caters for members in Rukwa, Ruvuma, Iringa and Mbeya regions.

The Northern Zone based in Arusha caters for members in Arusha, Kilimanjaro, Manyara, and Tanga regions.

The Eastern Zone which is based in Dar es Salaam serves members of Dar es Salaam itself, Coast, Lindi, Mtwara and Morogoro regions.

The Lake Zone that has offices in Mwanza region serves members of Shinyanga, Kagera, Mara and Mwanza regions.
